This exam is composed of a comprehensive medical exam work and medical history review, chest x-ray with B reader, pulmonary function test, OSHA respirator questionnaire, and any blood work as deemed necessary by the physician.
This physical exam satisfies OSHA Standard 29 CFR 1910.1001.

The purpose of this exam is to evaluate whether the current work responsibilities can be safely performed by the individual, and if not, it is used to determine what accommodations may be made in order for the individual to return to work.

A HazMat or "Hazardous Materials" Exam is an OSHA Physical Exam performed by a licensed physician for individuals who are exposed to chemicals or substances that can endanger an employee's health. It consists of a comprehensive medical exam work and medical history review, chest x-ray, pulmonary function test, OSHA respirator questionnaire, and any blood work as deemed necessary by the physician.

Use of OSHA Respirators include a 3-step process to certify use of a medical-grade respirator: (1) Completion of an OSHA Respirator Questionnaire (2) Completion of a Respirator Exam (if deemed necessary) to receive a signed Respirator Certification (3) Certified Respirator Fitting performed to confirm proper fit and use of a respirator, such as an N95.

The purpose of this exam is to evaluate whether the previous work responsibilities can safely be performed again by the individual post-recuperation, and if not, it is used to determine what accommodations may be made in order for the individual to return to work.

Both a Form F-2 and F2a attestation form are completed during this exam to indicate that the applicant is or is not physically able to perform the duties of a law enforcement officer as prescribed by DPSST.
This physical exam satisfies OAR 259-008-0010 (8) requirements.

An exam for individuals who are new employees at a company or who are starting at new positions in their company. It is used to assess the physical well being and their ability to complete the job they are being hired for. Its components vary by company, but generally consist of a hearing screening, vision screening, uranalysis, and basic physical evaluation. It may also include blood work, a drug screen, OSHA respirator evaluation, vaccinations and/or physical capacity evaluation.

This physical exam satisfies OSHA Standard 29 CFR 1926.1153 and 29 CFR 1910.1053.

This appointment is for individuals who were injured at work AND were seen at the ED/Emergency Room, Urgent Care, personal clinic, etc.,. Once scheduled, please note any medical facility you were previously treated at for this injury so we can get the necessary medical records to continue your care, or have your medical provider fax all records to our clinic (Tualatin: 503-563-5392, Hillsboro 503-596-2182) at least 24 hours before your appointment. Without these, your appointment may need to be rescheduled.
